What do I need to be seen for an Auto Accident Injury?
When coming into the office for your first visit, you will need to fill out our New Patient Intake Form and Auto Accident Injury Questionnaire. You will need the car insurance information of the other driver and yourself, including claim numbers, the names of your adjusters, and the phone numbers provided on your insurance card.
Our office does not typically provide 3rd party liability coverage. This means that after an auto accident, your visits will first be billed to your car insurance medical benefits.
